This will only appear once they have saved the eDischarge summary.   If the patient has been discharged and an eDischarge summary has not been completed, we cannot reactivate the summary. A letter will need to be dictated and sent to the Patient, their GP and a copy filed in the patient notes.   eDischarge Summaries with ‘Internal Use only’ should not be sent to the GP. Full process must be followed as above.   The discharge date will only print on the summary once the patient has been discharged on iPM. If this has not been done when the summary is printed, this needs to be hand written into the discharge date field at the top of the discharge summary.   The eDischarge Summary now also has version numbers printed at the top. All previous printed copies must be discarded in Confidential Waste and only the latest copy stored in the notes and sent to the GP.   Demographic details are pulled from iPM, if these are incorrect please update. This could mean an eDischarge Summary is sent to a GP for a patient not registered at their surgery. If the correct GP is not notified of a patients inpatient stay, they will not be able to follow up with the patient if required and could cause a patient serious harm.   Avoid copying and pasting large amounts of text from other systems into eDischarge. It will make the summary much longer than it needs to be. If you do paste small amounts of information in, please review it to ensure it is appropriate to include as the patient will receive a copy to take home with them upon discharge. This especially applies to the ‘Investigations carried out box’. Occasionally, Doctors will copy a whole radiology report which is not appropriate for a patient to view. You must note correctly where medication is being dispensed from so that the correct person can dispense items. If the information is incorrect (e.g. states that medication is due to be dispensed from Pharmacy but it is actually being dispensed from the Ward Pack TTA) then no one will be able to record that the medication has been dispensed to complete the discharge summary. You can add and edit information up to 2 working days post discharge using the Addendum tab. You cannot change the status of the discharge summary to ‘Ready for Dispensing’ until the summary has been signed off by the relevant Consultant/Clinician on the Clinical Information tab and saved (by clicking the Save Summary option). If you receive any error messages on the discharge summary it will be because some mandatory information may be missing e.g. if the summary hasn’t been signed off. These will need to be filled out before you can continue. Remember that you can ‘Revert Status’ on the discharge summary to make any additional changes but you can only do this before Pharmacy receive the drugs chart. You can discharge a patient with no medication. To do so you must record that the patient is ‘Ready for Discharge’ as soon as all clinical information is included on the Clinical Information tab and has been signed off. A discharge summary should be completed and sent to the GP within 24 hours of the patient’s discharge from Hospital. however, if needed, a discharge summary can be created up to 2 working days post discharge.
