A patient might be treated at their GP surgery or the local hospital.  Information is captured as a Detailed Care Record at the point at which treatment is provided.  This Detailed Care Record is accessible within a limited geographical area.  As well as this, a Summary Care Record of important details will be held on a national database called the Spine. These details will be both demographic (such as name, address and date of birth) and clinical (such as allergies, medication and some clinical correspondence).   The Summary Care Record is available to a wide variety of different Health Providers i.e. Acute Hospitals (ED, Pharmacy), urgent Care Centres & Minor Injuries Units, Mental Health, Community Service Teams etc.      Personal Demographic Service (PDS) role:  The Personal Demographic Service (PDS) role within the Summary Care Record allows users to view current and historical demographic information only i.e. name, address, date of birth, NHS number, GP etc.    Clinical information such as allergies, prescriptions, medications etc. cannot be viewed.
